Summer Camp | (Minor) 
Waiver, Release of Liability, Assumption of Risk, Indemnity Agreement and Media Consent 

1. PROGRAM DESCRIPTION

I understand that my child will participate in activities offered by Butterfly Underground LLC (the “Studio”), including but not limited to dance, movement, fitness, games, crafts, workshops, performances, and related activities (collectively, the “Activities”), whether on or off-site.  

2. ASSUMPTION OF RISK
I acknowledge that the Activities involve inherent risks, including but not limited to falls, collisions, physical exertion, emotional stress, and other risks that may result in injury, illness, or property damage. I voluntarily assume all risks, known and unknown, associated with my child’s participation.

3. RELEASE AND WAIVER OF LIABILITY
TO THE FULLEST EXTENT PERMITTED BY TEXAS LAW, I, ON BEHALF OF MYSELF AND MY CHILD, HEREBY RELEASE, WAIVE, AND DISCHARGE Butterfly Underground LLC, its owners, members, managers, employees, contractors, volunteers, agents, and affiliates (collectively, the “Released Parties”) FROM ANY AND ALL CLAIMS, DEMANDS, DAMAGES, OR CAUSES OF ACTION ARISING OUT OF OR RELATED TO MY CHILD’S PARTICIPATION IN THE ACTIVITIES, INCLUDING ANY CLAIMS ALLEGING NEGLIGENCE OF THE RELEASED PARTIES, EXCEPT TO THE EXTENT PROHIBITED BY LAW.
4. INDEMNIFICATION AND HOLD HARMLESS
I AGREE TO DEFEND, INDEMNIFY, AND HOLD HARMLESS the Released Parties from and against any and all claims, liabilities, damages, losses, or expenses (including reasonable attorneys’ fees) arising out of or related to my child’s participation, including claims brought by or on behalf of my child or third parties.

5. MEDICAL CONSENT AND EMERGENCY CARE
I certify that my child is physically able to participate. I authorize the Studio to obtain emergency medical care for my child if I cannot be reached. I agree to be responsible for any costs incurred for such care.

6. CODE OF CONDUCT
I understand that respectful, safe behavior is required. The Studio reserves the right to remove any participant for unsafe or disruptive behavior without refund, as permitted by law and policy.
7. PHOTO / VIDEO / MEDIA CONSENT (opt-out option available via checkbox on applicable registration page)
The Studio may take photographs, video, and audio recordings during Activities for promotional, educational, and social media purposes and I CONSENT to my child being photographed and/or recorded and grant the Studio a perpetual, royalty-free right to use my child’s likeness, image, voice, and name in any media without compensation.

9. ACKNOWLEDGMENT
I have read this agreement carefully and I understand the contents hereof. I acknowledge, agree and have executed this agreement voluntarily.
